Includes up to 90 Minute Initial Visit, Post Visit Research and Investigation, and up to 1 Hour Follow Up Visit where we will review and implement recommendations based on your unique needs.
The initial visit lasts up to 90 minutes and includes a detailed discussion of your chief complaints, health and family history. After Dr. Samuelson has reviewed your case, you will receive an individualized recommendation plan at your initial follow up appointment which will help guide you towards your health goals.
The initial visit and initial follow up are two separate appointments scheduled approximately 2-3 weeks apart.

This appointment is up to 1 hour in length. After your initial follow up visit, it is important to return regularly until health concerns are adequately managed. It is imperative to commit to and foster a working relationship with Dr. Samuelson in order to effectively help you. Expect anywhere from 3-6 visits in the first 6 months. This is the best way to ensure that your health goals are being met. Healing is a dynamic process and careful monitoring helps ensure our work together is successful. Written recommendations will be sent by Fullscript within 2 weeks of your visit. We offer a discounted price of $155 per follow up visit if you book your visits consecutively (within 6 weeks of your previous visit).

Payment and Cancellation Policy
PAYMENT:
Cash, checks and major credit cards are accepted and payment is due at the time of booking Initial Visits. Follow Up Visits require a credit card on file but will not be charged prior to your visit. There is an added 2.5% +$0.15 service fee for all payments by credit card. Initial Visit Service Fees are included in the quoted price. To avoid these fees please consider check or cash at the time of your visit.

CANCELLATION:
We require 48 hours notice to avoid being charged 1/2 the price of the visit unless promptly rescheduled. If you are ill we can change your visit to a telehealth visit or reschedule at the earliest availability.
Anything less than 24 hours prior may be charged full price. We will, however, do our best to avoid the full cancellation fees where possible. We understand unforeseen things can happen and also want to make sure everyone has access to available appointment times when they arise.
